J0B SUMMARY

The Food Service Manager is responsible for the overall management and operation of the department.

This includes budget control, personnel management, food production, sanitation, equipment

operation, and related clinical work.

The Food Service Manager participates in the performance evaluation of employees working under

his/her supervision. Responsible for implementing and applying the progressive discipline policy

where appropriate and is consulted on hiring and terminations in his/her area of supervision.

Food Service Manager is responsible for:

1. Hiring, training and supervising dietary staff.

2. Dietary food, supply and labor budgets.

3. Food purchases, production and service.

EDUCATION

High school diploma and Dietary Manager certification are required. Must be able to

communicate well verbally and in writing.

QUALIFICATIONS

1. Must be either a Dietary Assistant-, Certified Dietary Manager, or a Registered Dietitian.

2. Long term care o- acute care nutritional assessment experience desirable.

3. Long term care or related food service experience desirable.

4. Knowledge of federal, state and local regulations desirable.

5. Must have a pre-employment and an annual Tuberculosis screening examination.

RESPONSIBILITIES

1. Administration:

A. Plans, directs, coordinates and controls resources of the Food Services Department to

provide food service for guests, employees and residents.

B. Ensures compliance with corporate standards of operation, client and corporate

policies and procedures.

C. Maintains an effective working relationship within the facility.

D. Adheres to the operating budget or can satisfactorily explain variances.

E. Administers personnel policies and procedures to ensure fair and equal treatment of all

employees.

F. Procures, trains and directs employees within position allowance limitations.

G. Administers cost controls, records and clerical functions as required for the facility.

H. Maintains and adheres to the Food Service Policies and Procedures Manual and Managers

Handbook.

I. Ensures accurate preparation and timely completion of all required reports (financial and

other).

J. Assumes responsibilities as a department head within the facility. Attends appropriate

meetings within and outside the institution.

K. Administers Management Programs and communicates these to the facility and company

L. Delegates authority to supervisory staff for task details to facilitate smooth flow of

materials and services as appropriate.

2. Operations:

A. Ensures that approved menus are utilized according to corporate standards and agreement

specifications.

B. Ensures that all food is prepared, tested and served according to corporate standards.

C. Ensures that food production records are accurately prepared and standard recipes are used.

D. Performs and directs purchasing, receiving, storage, food production planning,

sanitation, corporate standards of operation, according to Policy and Procedures.

E. Supervises and/or directs resident services.

F. Ensures that resident records are complete and accurate at all times.

G. Ensures that an active and effective resident visitation and instruction program is

conducted on a continuing basis.

H. Maintains high quality food and food presentation programs.

I. Supervises and/or directs special event, and other special promotions as planned.

J. Maintains equipment to prevent losses to client or company.

K. Supervises and directs orientation, on-the-job training, in-service training, and continuing

education programs.

L. Acts on recommendations and instructions of Administrator, Corporate Manager, Corporate

Nurse, and / or Corporate Consultant Dietitian.

M. Recommends to the Administrator programs, equipment, procedures, and personnel to

improve service.

N. Assumes complete responsibility for the security of the kitchen (equipment and inventory)

and exercises control of a planned security program.

O. Administers or recommends a preventive maintenance program.

P. Administers safety program and documented safety review program within the Dietary

Department.

Q. Complies with all applicable regulatory agency codes.

3. Policies, Procedures and Programs:

A. Administers policies, procedures and programs within the established framework of

corporate operational guidelines.

B. Administers approved plans and procedures according to standards established.

C. Reviews and audits the programs conducted within the Food Service Department and

develops a plan of action to correct deficiencies and improve effectiveness.

D. Conducts corporate Quality Assurance audits.

4. Personnel and Training:

A. Motivates, develops and directs employees in an assigned unit.

B. Administers Food Services salary reviews and performance appraisals in accordance with

corporate policies for all Food Service Employees.

C. Directs the scheduling of all Food Service staff and employees.

D. Documents all training programs.

E. Provides information about new systems, methods, products and procedures.

F. Performs other tasks as assigned.

G. Attends required staff in-services, meetings, orientation, and continuing education activities as

required.

H. Is aware of and adheres to Patient’s Bill of Rights and confidentiality of patient information

including HIPAA regulations.

I. Interacts with patients and family members, co-workers, clinical and ancillary staff in a nonjudgmental,

supportive and calm manner.

J. Is aware of Patient Abuse Reporting Law.

Participates as assigned in the orientation of new staff and demonstrates a positive, helpful and

enthusiastic attitude.

5. Resident, Employee and Public Relations:

A. Promotes good public relations between the facility, corporate, and the community,

including areas of special functions and community relations.

B. Reports all resident complaints to the Administrator immediately.

C. Establishes and maintains effective working relationships with the facility department

heads, residents, employees and suppliers.

D. Keeps the Administrator informed of progress on projects and any other information

that might affect services to the residents and employees of the facility.

E. Performs all other duties as assigned.

6. Clinical and Related:

A. Review and assess all residents for nutritional status.

B. Develop specific and appropriate nutritional interventions and implement.

C. Document nutritional interventions in designated clinical records.

D. Participate in Interdisciplinary Care Planning.

JOB SUMMARY

The Cook is responsible for the preparation and serving of the residents' meals using proper food handling

techniques. In the absence of the Food Service Manager, the cook on duty is the person in charge of the

department.

RESPONSIBILITIES

1. The Cook is responsible for the preparation of the meal according to the menu and census.

2. The Cook is responsible for the supervision of dietary aides in the absence of the Food Service

Manager.

3. The Cook is responsible for the set-up of paper work for new residents in Manager's absence.

EDUCATION

Must be able to read, write and follow oral and written instructions.

QUALIFICATIONS

1. Six months experience preferable in institutional food preparation or related field.

2. Physically able to carry out duties of the Cook (must be able to lift 25 pounds).

3. Knowledge of and ability to use Food Service equipment.

JOB SUMMARY

He/She is responsible for the care of the residents in a designated area of the facility including the administration of medications as ordered by the residents’ physician. He/She communicates with off going and arriving shifts of personnel to assure the continuum of quality of care. Makes rounds, evaluating the physical, emotional and social needs of the residents making sure these are being met through the staff assigned to work with each group of residents.  Assesses assigned staff's ability to provide for each resident's needs and provides intervention as indicated. Supervises the care provided by the Nursing Assistants assigned to the unit, providing intervention, and education as indicated. Makes adjustments in assignments as indicated. Prepares and administers medications and treatments according to procedures. Consults with physicians, receives, records and transcribes orders as required. Makes rounds with physicians when necessary. Reviews resident's care plans and assures these plans are followed. Documents on records as per policy and procedures of the facility. Participates in the evaluation of the employees working under his/her supervision. Participates in staff meetings and in-services as required.

The Registered Nurse, Licensed Practical Nurse or Licensed Vocational Nurse is responsible to the Director of Nursing and/or the Assistant Director of Nursing.

EDUCATION

He/She must be a graduate of an accredited school of nursing currently registered with the state agency for nursing licensure and hold a valid nursing license in the state he/she is employed.

RESPONSIBILITIES

1.      Receives report from the off going shift.

2.      Reviews communication systems to be knowledgeable of any unusual occurrences during the previous shifts.

3.      Assigns Nursing Assistants and other designated personnel assuring each resident's needs will be met during that shift.

4.      Gives report to Nursing Assistants.

5.      Makes rounds of the assigned unit frequently during the shift providing intervention as needed. During the entire shift, will be attune to the needs of the unit while doing other nursing tasks.

10.    Continually observes progress and quality of work of the Nursing Assistants to assure that residents are turned, repositioned, toileted, walked, exercised, provided food and fluids, well groomed, involved in their specific restorative program, provides intervention as indicated, etc. Also observes the resident's environment to assure a clean and safe environment is provided for each resident.

11.    Document on nurse's notes as per policy. Assures there is accurate and timely documentation of any unusual observation, incident, or happening.

12.    Admits residents as required.

13.    Assists in the ordering and reviewing of results of laboratory tests as indicated. Notifies physician of results.

14.    Notifies the physician of unusual findings as indicated.

15.    Assists with auditing charts.

16.    Assists with answering and correcting consultant reports.

17.    Reviews, signs and provides intervention as indicated for Nursing Assistants documentation.

18.    Counts required medications with on coming and off going shif^ts.

RESPONSIBILITIES

It is to be understood that the Nursing Assistant works by general, consistent, pre-arranged "Primary" group assignments as the basis for each shift worked, but must be able to accept changes and adapt to added assignment as the need arises. In addition, each Nursing Assistant must work with each other to assure safe practices for each resident and staff are followed.

1.      Signs in per facility protocol.

2.      Checks residents and makes rounds on assigned group noting significant issues for intervention. Begin planning time to assure all resident's needs are met throughout the shift.

3.      Assists residents as indicated to provide A.M., P.M. and daily care to assure residents are clean, dry, toileted, continent, well-groomed and cared for in the manner they deserve. Follows restorative plan as outlined on the Interdisciplinary Care Plan. Provides for privacy and dignity at all times.

4.      Assists residents to meals. Assists residents to toilet prior to meals. Assists resident to wash hands and face, brush teeth, put on glasses, get to dining room and in good position prior to meal.

5.      Provides assistance and restorative eating as indicated. For those residents who are the most debilitated, consider their weakness in planning time for the whole group of residents.

6.      After meal, assist resident to wash hands and face, return to their room and toilet as needed.

7.      Assists in dining areas as assigned. Offers assistance to residents as needed. Offers substitutions for items not eaten well by any resident. Promotes atmosphere conducive to increased nutritional intake for each resident in the dining room.

8.      For those residents unable to go to the dining room, assists in service, assisting and supervising meal service as assigned.

9.      Assists in transporting used trays to the kitchen following appropriate infection control policies and procedures.

10.    Assists residents to plan their time to enable them to be well cared for as well as attend their desired functions during the shift. (i.e. activities, visits with each other and their families, outings, meals, quiet times, etc.)

11.    Answers call lights timely assisting with care as indicated. Keeps call lights available for the residents.

12.    Encourages and motivates residents to participate in recreational activity of body and mind.

13.    Provides and keeps fresh water and fluids available for the residents at all times. Offers fluids at least every 2 hours or each time in the room, when appropriate.

14.    Timely serves and provides assistance with between meal nourishments as indicated.

15.    Follows bath and whirlpool schedules as outlined for each resident. Do nail care, wash hair, shave, etc. to assure personal hygiene practices are always followed.

16.    Checks all residents frequently and at least every 2 hours assuring the resident is repositioned, turned, toileted, clean, safe, etc. Provides intervention as needed to provide a safe clean environment.

17.    Checks all residents daily for condition of skin, reporting findings to Licensed Staff assigned to unit.

18.    Assists licensed staff when needed to position residents in need of treatments.

19.    Keeps hand rolls and positioning devices in place as needed and as indicated for each resident.

20.    Assists residents to bed for naps or night sleep as needed. Provides a quiet restful and safe atmosphere when residents are resting.

21.    Takes and records vital parameters as assigned. Assists with weekly and monthly weights as assigned.

22.    Cleans and properly arranges all bedroom furniture to include keeping the residents orderly, clean, and following infection control guidelines.

23.     Follows Plan of Care.

24.     Reports any changes in condition or any resident/family concerns to licensed nurse.

28.    Follows appropriate infection control practices in working with the residents and equipment.

30.    Records information on the Nursing Care Records timely.

31.    Keeps Licensed Staff informed of pertinent information timely. Writes pertinent information on the Group Assignment Sheet and Communication Books.

32.    Provides information accurately and timely for information to be added to the residents Interdisciplinary Care Plan.

33.    Cleans ancillary rooms and equipment as assigned. Assists other members of the staff to keep each area of the facility clean, orderly, safe, and a homelike atmosphere for the residents.

34.    Assists other staff members as needed throughout the shift. Cooperates with other personnel to be sure residents are of prime importance to all members of the staff.

35.    Assists new staff members in orientation as needed throughout the shift.

36.    Maintains safe environment at all times, wiping up spills, working with housekeeping staff when available picking up articles as noted, etc.

37.    Gives report to Licensed Staff and turn in completed Group Assignment sheet prior to leaving the unit to go off duty.

38.    Performs other tasks as assigned.
